不勉強にも、For the first ear でした。Vibrio vulnificus は鉄依存性なので、ヘプシジン低下により腸管からの鉄吸収が促進している肝硬変患者やヘモクロマトーシスや鉄剤の常用などで血清鉄が高い患者が発病しやすいのだそうです。

第44回："A Creeping Suspicion"
初めて知ったのですが、ヘルペスというのは、「這う」という意味のギリシア語に由来するのだそう。どんな組織でもそうですが、仲間外には通じない隠語を作りたがります。医学用語に関しては、その素材としてギリシア語やラテン語を用いることが多いわけです。ちょうど日本語が格式高く聞こえるように漢語を使って専門用語を作るのと同じことです。

A 51-year-old woman was brought to the emergency department by her husband after the development of nonsensical speech, including confused word choices, difficulty naming objects, and perseveration with counting. Her orientation, comprehension, and speech patterns had fluctuated throughout the morning. Her husband noted that the day before presentation, she had reported headache, nausea, vomiting, and fatigue. No fevers were noted. She had no history of similar symptoms.
"confused word choices" には、"verbal paraphasia"という用語がありますが、内科医にとっては普通の言葉ではないので、上記の表現を用いたのでしょう。
Given the very high sensitivity of two successive qPCR tests, infection with HSV, VZV, or enterovirus is now even less likely. In addition, the patient has had no improvement in her speech. The workup should be expanded to include less common infectious processes that can cause CNS disease, including tuberculosis, cryptococcosis, toxoplasmosis, arbovirus infection, human herpesvirus 6 infection, cytomegalovirus (CMV) infection, and human immunodeficiency virus (HIV) infection.
脳炎の原因としては、HSVが最もコモンなわけですが、それが陰性のとき、何を考えるのかということが挙げられています。
When the preponderance of clinical evidence points to a particular diagnosis and treatment entails minimal risk to the patient, especially if failure to provide treatment is associated with high mortality, empirical treatment is appropriate even as alternative diagnoses are being considered.
今回のケースで最も大切の事のような気がします。ある病気の可能性があってそれに対する治療のリスクが少なく、その診断が正しくて治療しなかった場合に死亡率が高ければ、経験的治療は適切であるということ。つまり、コンテキストにより正しさは変わり得るということ。

The documentation of quinine-dependent antibodies supports the identification of quinine as the cause of the patient’s TMA.
ADAMTS13は、TTPの診断に必須ですが、コマーシャルベースでは検査できないようです。
第42回："A Gut Instinct"
タイトルは、 "gut instinct" で「本能的直感」という意味があるので、腸結核と掛けたのでしょう。今回からどんな理由であれ、他の参加者にお伝えしたいことを３箇所選んで参加し、その理由をお話頂くというスタイルに変更しました。

この部分を多くの先生が挙げました。
しかし、実際感染した場合は、「史記 南越伝」に「因禍為福、成敗之転、譬若糾纆（禍によりて福となす、成敗の転ずること、譬れば糾える纆のごとし）」とあるように、ガツガツせず、ちょっとした休暇と考えてみてもいいかもしれません。
第41回："Gotta catch’em all! Pokémon GO and physical activity among young adults: difference in differences study"
今回は、クリスマスということもあり、BMJのクリスマス記事から「ポケモンGO」ネタ。

感想：
- ネタはふざけているが、差分の差分分析を使った真面目な研究。なんと言っても、ソーシャルキャピタルで高名なイチローカワチ先生が名を連ねている。
- 「ポケモンGO」は、歩行を促す効果はあるが、WHO推奨の半分程度で、持続期間も6週間と限定的。
- i-Phone6利用者が対象という時点でバイアスがかかっている気がしますが、クリスマス記事に目くじらを立てるのも大人気ないでしょう。
- 日本では、「人の噂も七十五日」と言いますが、アメリカ人は日本人より飽きっぽいのかしらん。（実際、身近にまだ頑張ってやっている方が複数いらっしゃいます！）
