タイトルは、 "gut instinct" で「本能的直感」という意味があるので、腸結核と掛けたのでしょう。今回からどんな理由であれ、他の参加者にお伝えしたいことを３箇所選んで参加し、その理由をお話頂くというスタイルに変更しました。

しかし、実際感染した場合は、「史記 南越伝」に「因禍為福、成敗之転、譬若糾纆（禍によりて福となす、成敗の転ずること、譬れば糾える纆のごとし）」とあるように、ガツガツせず、ちょっとした休暇と考えてみてもいいかもしれません。

第41回："Gotta catch’em all! Pokémon GO and physical activity among young adults: difference in differences study"

今回は、クリスマスということもあり、BMJのクリスマス記事から「ポケモンGO」ネタ。

感想：

• ネタはふざけているが、差分の差分分析を使った真面目な研究。なんと言っても、ソーシャルキャピタルで高名なイチローカワチ先生が名を連ねている。
• 「ポケモンGO」は、歩行を促す効果はあるが、WHO推奨の半分程度で、持続期間も6週間と限定的。
• i-Phone6利用者が対象という時点でバイアスがかかっている気がしますが、クリスマス記事に目くじらを立てるのも大人気ないでしょう。
• 日本では、「人の噂も七十五日」と言いますが、アメリカ人は日本人より飽きっぽいのかしらん。（実際、身近にまだ頑張ってやっている方が複数いらっしゃいます！）

第40回："A Tight Predicament"

タイトルの、"tight" は、"stenosis" に通ずるのでしょうか？それにしても、"predicament" の意味が覚えづらい。語源で考えると、「前もって言われていたこと」のような意味のはずなので、（言ったこっちゃない）苦境みたいに派生したのでしょうかねぇ。

感想：

• 途上国では、まだまだ多い病気。MSの原因は、100％リウマチ性弁膜症ですから。一方、日本では診ることが少なくなって診断能力が落ちているかも。
• 「【CD-ROM付】ティアニー先生の心臓の診察 」によると、僧帽弁狭窄症の拡張期ランブルはベルで聴く、聞こえる範囲は100円玉の大きさとのことで、聴診は難しい。むしろ、OSの聴取が大切とのこと。
• エコーを当てて、僧帽弁の"hockey stick"を見つけるのも、有用。

第39回："Scratching Below the Surface"

タイトルは、scratch the face で、「〔物の〕表面を引っかく、表面に傷をつける、 〈比喩〉上っ面をなでる、表面的に論じる、〔研究・学習などを〕ほんの少し学ぶ［かじる］、 〔やるべきことの〕始まりにすぎない」などの意味があるそうで、"below"を加えることで、その裏を理解するということを言いたいのでしょう。表面抗原の"Surface"も含意しているのかもしれません。

• 「脳に棲む魔物」(Brain on Fire) というタイトルでニューヨーク・ポスト記者の罹患・回復の記録した本が日本語に訳されているようです。
• 上記本を映画化したものが、今年９月１４日公開されたそうです。来年にはDVDやAMAZONプライムで見れるようになるかもしれません。

ドラッカー読書会と我々の抄読会

　 右に載せた本の著者吉田麻子さんの出版記念講演に出かけてきました。私自身は、別にドラッカリアンというわけではありません。むしろ最近まで名前を知ってはいたものの、単純に「効率主義者の神」くらいの認識でした。その認識を変えてくれたのが、安富歩先生の「ドラッカーと論語 」で、単純な経営学者ではないことを知ったことが、今回の会に出向くきっかけになりました。さらに、その会場が、木田保造率いる木田組が建てた旧日露漁業株式会社の社屋だったニチロビルにあるMARUSENだったということもあります。

　肝心の会は、著者の講演に、ドラッカー読書会のデモ、懇親会と続きました。読書会は、函館でも、右掲本の下敷きとなったドラッカーの「ドラッカー名著集1　経営者の条件 」を素材に、みなで同じ部分を読んで、気になったところにラインを引き、読書会当日は、月１回３時間の時間で、各参加者がラインを引いたところを読み上げ、なぜそこにラインを引いたかを説明し、ファシリテーターがコメントするというやり方のようです。

　私達の抄読会の持続可能性（サステナビリティ）を高めるためのヒントになりました。今後、抄読会のなかで徐々にアイディアを提起していきたいと思います。

第38回："Against the Grain"

タイトルは、「木目に逆らって」という意味の成句と「穀物に対する抗体」という病態生理を掛けたのでしょう。

• Celiac disease is an autoimmune disorder that affects the small bowel and that is triggered by ingested gluten from barley, rye, and wheat..
• Celiac disease is also manifested outside the gastrointestinal tract. Rashes (e.g., dermatitis herpetiformis), arthralgias, neurologic and psychiatric symptoms, fatigue, and infertility can be presenting manifestations.
• The diagnosis is usually made on the basis of serologic screening, followed by a confirmatory small-bowel biopsy. The serologic test of choice is the IgA anti–tissue transglutaminase antibody assay, which is highly standardized, specific (94%), and sensitive (97%).

感想：

• 直球ケースでした。でも、有名な割には、診たことがなく、日本人での疫学は、どうなんでしょう？
• ジェローム・グループマン著「医者は現場でどう考えるか」は、何を食べても吐き出してしまう症状に苦しむ３０代の女性のケースで始まる。１５年に渡り３０人近い医師の診療を受けながら、「摂食障害」と見なされてきたが、先入観抜きで彼女の「物語」に耳を傾けた医師がセリアック病を診断し、彼女を救う。いかに人間が認知バイアスから抜け出すことが難しいかを示す好例である。
• セリアック病の名前が紛らわしい。（→「coeliac という単語はギリシャ語: κοιλιακος, koiliakos（腹部の）に由来しており、古代ギリシャにて本疾患を記述したとされるカッパドキアのアレタイオスの著作の翻訳に基づき、19世紀に導入された」らしい。）
• 肝心の検査が日本ではコマーシャルベースではないので、疑ったら、GFDを指導して、治療的診断ということになりそう。麺類、パン、ビール、醤油…を除去なんて、つらい。
• "Celiac Disease: Ten Things That Every Gastroenterologist Should Know" に要点がまとまっている。
• ある資料によると、皮膚病変は、「肘、膝、臀部、肩、頭皮の伸筋領域に対称的に分布する強いかゆみを伴う丘疹小水疱性発疹である疱疹状皮膚炎を起こす。」とのこと。
